90791: Initial session cost: $250.00
90837: Psychotherapy cost: $220.00
Assessment: Costs vary on a case-by-case basis, but not to exceed $2,000.

I expect that your care will require approximately 8-10 weekly therapy sessions. Assessments will require approximately 3-5 sessions of various lengths. Although the total number of sessions required to meet your goals are unknown at this time, the average duration of treatment for a similar diagnosis is 8-10 session. Depending on the progress that we make, I expect that you will need [8-10] sessions. At $220.00 per session, the estimated cost would be approximately $1760 – $3330.  If you are utilizing insurance benefits, the cost to you is subject to your individual contract with your insurance company and dependent on your coverage details.  Please contact your insurance company and acquire additional information regarding your deductible, copay, and any additional out-of-pocket costs. Preauthorization may be needed. If it is not obtained, you will be responsible for the costs of services.
